The knee is the largest joint in the body and it is also one of the most complex. The knee joint is made up of four bones, which are connected by muscles, ligaments, and tendons. The femur is the large bone in the thigh. The tibia is the large shin bone. The fibula is the smaller shin bone, located next to the tibia. The patella, otherwise known as the knee cap, is the small bone in the front of the knee. It slides up and down in a groove in the femur (the femoral groove) as the knee bends and straightens.
The hip is one of the main weight-bearing joints in your body. It consists of two main parts:
·A ball (femoral head) at the top of your thighbone (femur)
·A rounded socket (acetabulum) in your pelvis
Ligaments, which are bands of tissue, connect the ball to the socket and help keep the ball and socket steady. A smooth, tough material called articular cartilage, which cushions the bones and lets them move easily, covers the surfaces of the ball and socket. All the rest of the surfaces of the hip joint are covered by a thin, smooth tissue liner called synovial membrane, which makes a small amount of fluid that acts as a lubricant so that the bones in the hip joint will not rub against each other.
Shoulder Anatomy
The shoulder is a ball and socket joint. The shoulder joint comprises bone ligaments tendons and muscles that connect the arm to the torso.

The shoulder has 3 joints that work together to bring about movement in multidirectional are-
1. Acromioclavicular joint - The AC joint gives us the ability to raise the arm above head.
2. Glenohumeral joint – This is a ball-socket type joint. The Ball is the top rounded part of humerus and the Socket is bowl shaped part of scapula called the glenoid into which the ball fits. The joint allows the arm to move in a circular rotation as well as towards and away from the body.
3. Coracoacromial joint
The bones of the elbow are the humerus (the upper arm bone), the ulna (the larger bone of the forearm, on the opposite side of the thumb), and the radius (the smaller bone of the forearm on the same side as there is a second joint where the end of the radius (the radial head) meets the humerus. the thumb). radius has to rotate so that you can turn your hand palm up and palm down. At the same time, it has to slide against the end of the humerus as the elbow bends and straightens.
Laminectomy is surgery to remove the lamina, two small bones that make up a vertebra, or bone spurs in your back. The procedure can take pressure off your spinal nerves or spinal column.
Nucleoplasty is a minimally invasive procedure for percutaneous disc decompression that uses radiofrequency based technology to ablate and coagulate soft tissue to decompress contained herniated discs. This causes the disc herniation to reabsorb, taking pressure off the exiting spinal nerves.
Percutaneous disc decompression techniques have been used since the late 1970s to provide a minimally invasive method of decompressing the nucleus pulposis, thereby depressurizing disc herniations, and reducing the compression of the disc herniation on nerve roots and other spinal structures. Various methods have been devised to accomplish the decompression and subsequent reduction in pain including the use of mechanical, chemical, laser, radiofrequency, and ultrasound.
The aim of operation is decompression of the nerve root. Under general anesthesia a 1 inch incision is made in the low back overlying the nerve root. Using the operative microscope a small crescent of bone is removed from the spine, exposing the nerve root and herniated disc material (laminotomy). The disc material compressing the nerve root is removed and the underlying central disc space is curretted free of retained, degenerated disc nucleus (discectomy).
Spinal fusion is a surgical procedure used to correct problems with the small bones of the spine (vertebrae). It is essentially a "welding" process. The basic idea is to fuse together the painful vertebrae so that they heal into a single, solid bone.
Spine surgery is usually recommended only when your doctor can pinpoint the source of your pain. To do this, your doctor may use imaging tests, such as x-rays, computed tomography (CT), and magnetic resonance imaging (MRI) scans.
A boon to senior citizens with osteoporotic spinal fractures
As we go older our bones start becoming brittle. The calcium and mineral content of the bone starts reducing. The amount of bone tissue in the spinal vertebrae reduces and makes senior citizens prone to get fractures of vertebrae even without accident or fall.
The person comes to a doctor with acute back pain, hunched back, inability to sit or stand and perform routine daily activities.
Earlier, there were only 2 options. One was to take strict bed rest for 6 to 8 weeks, wear bulky belts or braces and wait till the fracture heals. Once an elderly person is confined to bed he or she is prone to get bad complications like bedsores, pneumonia, infections etc. Also once they are confined to bed they have episodes of depression, which is understood as he / she has to depend on other people even for toilet purposes with nuclear families it is not possible for them to have attendants as well.
The second option was an open operation, which carries lots of risk at that age, & it is quite costly also. Vertebroplasty & kyphoplasty are the newest minimally invasive treatment options for such needy patients.
In Vertebroplasty after confirmation of diagnosis on X-ray, MRI or C.T. Scan patient requires a day’s admission in hospital.
Osteoporosis means parous & weak bones. This leads to structural deteroration of the bone. It predisposes a human being to fractures, deformities due to fragility of bone.
Osteoporosis is a major public health threat for approximately Indians 80% of whom are women. In U.S. today 10 million people are already diagnosed to have osteoporosis. Unfortunately in India the awareness about the disease is not there in community as well as many medical practitioners. The incidence is so high that one out of every two women and one in eight men over 50 will have an osteoporosis related fracture.
