How does LipoSelection ® only by VASER ® work?

What happens during the LipoSelection ® procedure?
First, the area to be sculpted is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). What is a tummy tuck?
A tummy tuck is a surgical procedure that addresses the problem of loose or wrinkled skin on the abdomen due to pregnancy or excessive weight gain. This procedure is available for both men and women. Tummy tuck is the removal of excess skin from flabby tummies, mostly due to stretchmarks.
Who is a candidate for a tummy tuck?
Anyone who has loose, excessive skin on their bellies due to the aforementioned situations can be a viable candidate for a tummy tuck.
What kind of tummy tuck does Roura DermSurgery offer?
Roura DermSurgery offers lipo-abdominoplasty, a tummy tuck procedure that is different from the usual.
How is it performed?
In a standard abdominoplasty, the skin in the fatty area is first undermined, and then the excess skin is carefully sliced away. The areas are then sutured together, and drains are installed. These drains are usually kept in place for 2-3 days.
In a lipo-abdominoplasty, Roura Dermsurgery performs a VASER liposuction first to give the patient their desired abdominal contours. The excess skin is cut away afterwards. There is, however, no need for either undermining or drains.
What is undermining in abdominoplasty?
Undermining means releasing the attachments of the subcutaneous tissues from the abdominal muscles then pulling down the skin and cutting off excess skin. This is quite dangerous due to possible bleeding and loss of blood supply to the tissues that can cause necrosis.
What are abdominoplasty drains, and how do they work?
After a standard abdominoplasty with undermining, drains need to be placed, that is small tubes are left inside the layer of undermining to prevent accumulation of fluid and blood in the space created by the undermining the tissues, this is done to prevent seromas (serous fluid accumulation) or hematomas (blood accumulation).
Why does Roura DermSurgery offer lipo-abdominoplasty?
Although most surgeons do not advocate this procedure due to its inherent risks, at Roura Dermsurgery, this is safely performed because of VASER technology. This allows us to perform the procedure in a safe and effective manner with less pain and recovery time involved.
How long does the procedure take, and what preparatory timeframe/actions will I need?
The procedure would take around 3-4 hours this is already inclusive of the time for the VASER-lipo and the removal of excess skin. Rest and light activities are suggested for the first week.
What can I expect after the procedure?
After the surgery, you be bandaged and bound for the recovery period. There will be some drainage for a few days. The area will feel a bit sore and tender to touch.
You will be up and about immediately after the procedure, however; you are not required to be bedridden or immobile. Normal activities can be tolerated, for as long as they do not cause excessive stress on your abdomen. Swelling is expected, maximal during the first 2 weeks, then slowly subsiding over the next several months. The final form of your new abdominal contours can be expected within 3-6 months.
Will there be scars?
Yes, there will be scars from the procedure. These scars will be visible on your tummy, usually on the lower part of the pubis, hidden in the garter line. The scars will initially be red, but then they will eventually fade.
Butt Augmentation
A beautiful front and behind make the body curvaceous. Butt augmentation can be carried out with either bio-compatible material or with one’s own fatty tissues. In our center, we advocate the use of Autologous fat transfer for butt augmentation. With this procedure, fat is harvested thru a liposuction. It is processed is such a way that it can survive once transplanted into the gluteal area. Swelling is expected. There may be a percentage of fat absorption by the body, however, there will be a significant volume that will be retained to increase and enhance the butt.
Breast Augmentation
Asian women are predominantly small-breasted. Massages, creams, exercise, breast enhancing bras, foam padding have all been used to enhance the female breasts.
Now, women have a choice. Silicone or Saline-filled breast implants or Autologous fat transfer.
Breast implants are inserted via surgery behind the patient's breasts. The incision placed either around the areola (peri-areolar), at the inframammary crease (inframammary) or thru the armpits (trans axillary approach)
For Autologous Fat Transfer, your own fat is harvested through liposuction and then processed to be injected into your breasts. While silicone implants are permanent, fat injections into the breasts has a significant rate of absorption which only mildly increases patient’s breast cup. In case of full fat absorption, either more fat can be injected to the breasts or breast implants are added.
Breast Reduction and Breastlift
Breast Reduction is for women who are overly endowed women but are bothered by their beasts for aesthetic or health reasons. For sagging breasts, BreastLift is the solution.
If sagging is caused by heavy breasts a patient needs a combination treatment. Roura DermSurgery uses VASER Breast Reduction, called Liposelection, to remove fat from the breasts. Breast tissue is left intact to retain its sensation. Lactation and breastfeeding remain normal. Depending on the breast skin tone, this procedure may leave her skin scarless or may have a minimal inconspicuous scar around the areola.
Gynecomastia
Gynecomastia is the enlargement of the male breast. Although a big muscular chest is always desired, large fatty breasts on a male chest is not desirable. This is a common source of embarrassment causing a low self- esteem for most men. They avoid taking their shirt off to go swimming and wearing of tight shirts makes it more noticeable.
Now this problem can be treated easily with Liposelection only by VASER. Fatty tissues of the male breast are taken out via small inconspicuously place incisions to decrease and flatten the chest. This leaves a more defined pectoral muscle.
Face
Facial Surgery

Upper Eyelid Surgery (Eyelid Deep Setting / Double Eyelid Creation)
This procedure entails removal of excess skin on the upper eyelids due to aging, or basically due to genetics, giving the patients the aged, or sad and tired look.
Upper Blepharoplasty also addresses the creation of double eyelid in cases of oriental eyes with no upper lid crease. In doing so, patients acquire a fresh, younger look with bright sparkling eyes.
Done under local anesthesia, the surgery takes out excess skin and sutures it back in place with a special technique that creates a new fold. Swelling and bruising are maximal during the first 2-3 days and eventually subside. Sutures are taken out after 1 week.
Eyebags Removal
Excess fat on the lower lid gives the eye a puffy look. For young patients in their 20's or 30's, a Subconjunctival approach is used in which fat can be taken out without cutting the skin.
For older patients above 40's, the Transcutaneous approach essentially takes out both fat and excess skin. A special technique, under local anesthesia, can also eliminate the hollow-eye look. Swelling and bruising are maximal during the first 2-3 days and eventually subside. Sutures are taken out after 1 week.
Noselift (Rhinoplasty)
Asians have predominantly flat noses or the wide-based noses. And many have desired to improve the Oriental nose. A combination of a clinical eye and an aesthetic sense is required to create a beautiful nose. For a natural look, Asians should not have high bridge pointed noses like the Caucasians.
The Noselift is generally done to enhance the nasal bridge. A synthetic biocompatible material is carved and molded to suit the patient’s face and then inserted in the nasal dorsum to increase the nasal height.
For patients with wide-based noses, the Noselift can be combined with a Nosetrim. Excess nasal tissue is removed and sutured back into place to create a trimmer nasal base opening. This is done under local anesthesia. Swelling and bruising are maximal during the first 2-3 days and eventually subside. Sutures are taken out after 1 week.
Facelift and Browlift
With aging, the face starts to show wrinkles, and later on its skin also starts to sag, and then folds even appear on the neck. These are the effects of gravity on the facial structures – which can be effectively treated with a Facelift. . Facelift gives a younger definition of the face. Incisions are placed into the hair to hide the scars later. Tissues are pulled upward and excess skin is removed, after which it is then sutured back in place.
Facelift can be combined with fat transfer to address facial volume loss due to aging. This is done under local anesthesia with or without sedation. Swelling and bruising are maximal during the first 2-3 days and eventually subside. Sutures are taken out after 7-10 days.
For a full facial rejuvenation, Facelift and Facial Fat Transfer can also be combined with Blepharoplasty and Rhinoplasty.

Radiage – Radiofrequency Skin Tightening
Radiage is a new treatment that provides skin tightening without surgery. This treatment uses the application of radiofrequency energy (radio waves) to cause heating of the collagen in the skin. This results in shrinkage of the collagen, leading to a mild tightening of the skin. Radiage is useful for individuals in whom there is a degree of skin wrinkling but not significant skin looseness.
Multiple treatments, usually 4 or 5, are required and these are administered every 3-4 weeks for best results. These feel warm but are not uncomfortable.
Treatments take about 30 minutes and may cause a mild redness of the skin that can last for a few hours. No other side effects have been reported with this technology, making it the safest on the market.

Microneedle Therapy
Collagen is the structural backbone of the skin. All procedures done for skin rejuvenation is to stimulate collagen production - whether it is by the use of skin care products, facial treatments, lasers, radiofrequency, etc.
Aging is characterized by a loss of collagen and all means of treatments are targeted with the increase in collagen of the dermis of the skin.
One such treatment is the Microneedle therapy that uses the Dermaroller. With the Dermaroller, collagen formation is stimulated by producing controlled injury of the skin up to the dermis where collagen is needed.
In order to do this, multiple needles of appropriate length are rolled into the face, hundreds of small needle sticks produce micro-injury to the skin. The skin is left intact and is not damaged. The skin’s response to this kind of injury is collagen formation and thus results to rejuvenated skin.
This procedure is carried out under topical anesthesia; the skin will be slightly red for the first 2-3 days. Treatment may be repeated after 2-4 weeks.
Laser Hair Removal
What is IPL Hair Removal System?
It's a non-invasive light therapy device specifically designed to eliminate undesired hair from all parts of the body. Different parameters (fluency, pulses number up to 3 during the same shot, light and pause duration, handle cooling temperature) offers a complete range of protocols for each type of hair (color, size) and skin (photo types)
How does IPL remove hair?
IPL uses pulsed light to remove hair and impair hair regrowth. First, a cool gel and a hand-held treatment unit are gently applied to the skin. Then, in a flash, the light is conducted on the hair follicles by the melanin of the hair. It safely penetrates the skin and disable all hair follicles simultaneously.
What is IPL treatment like?
When IPL's pulse of light is delivered to the hair, (one or up to three pulse depending of the hair and skin type) the cooling system of the handle avoids you to feel a mild sting or pinch. No local anesthesia or pain medication is required, and in case of skin redness a dermatological cream can be used.
How many treatments are required?
Several treatments may be needed, with each session usually lasting 10-30 minutes, depending on the size of the area to be treated. Several treatments are also needed because the hair have different growing phases. The number of treatment varies upon your hair color and location on the body.
Are there any life restrictions after one treatment?
No. You can return to work the same day and resume all regular activities. You should of course limit sun exposure, which is always a good idea.
Botox
Botulinum toxin is injected into certain hyperactive muscles of the face to soften wrinkles. It can be used for horizontal forehead lines, crow’s feet, glabellar frown lines. Eliminating these lines makes the face look younger and fresh and renewed. Advanced techniques with Botox can soften the gummy smile, the frowning face, the square jaw,. It can even do a non-surgical facelift with the Dermalift technique used in Botox. After injections, the full effect will be seen within 2 weeks and the effect will last anywhere from 3-6 months after which you can have another treatment.
Fillers
As one ages, lines and wrinkles begin to appear. These lines and wrinkles may be due to hyperactive muscles or to volume loss . Fillers are made of Hyaluronic acid or other biocompatible materials that can augment facial features. It is injected into the skin to fill up wrinkles, create fuller cheeks, and augment the chin. It can also be used to temporarily augment the nasal bridge. Depending on the kind of filler used. Results can be temporary (6-12 months), longer lasting (1-2 years) or permanent.
Acne Scar Revision
Control of acne is not enough. Acne scars not only leave permanent facial marks but also marks one's self-image and well-being. Several treatments can be used to address the problem of acne scars - usually a combination of treatment works best.
Treatment options include (1) Chemical peels to slowly peel off the surface of the skin layer by layer (2) Microdermabrasion can go beyond a regular peel to address deeper scars. (3) CROSS technique addresses the so-called ice-pick scars which are known to be the most difficult to treat (4), Subcisions or subcutaneous incision are used to release the fibrous bands holding the scar down to make the scars shallower and (5) Microneedle therapy is used to stimulate collagen production for full face facial rejuvenation. Proper assessment of condition is needed to give individualized treatment for different skin conditions.
Scar and Keloids
Different kinds of scars require different treatments. Scars that deal only with a change in the color of the skin may require only bleaching or chemical peels. Scars that disturb the integrity of the skin leaving jagged or linear scars either from surgery or injury shall be treated accordingly.
Keloids and Hypertrophic scars are skin conditions wherein there is proliferation of the car tissue leaving large, hard, elevated scars which may extend even outside the original site of injury. Treatment options include Dermabrasion, Intralesional Steroid injections, Scar Revision, Silicone sheets etc.
Body Bleaching
Body Bleaching can be carried out to lighten dark spots. Skin lightening agents are applied on the skin for a certain period of time before seeing results. Treatments are enhanced by combining Topical medications with Body Peeling, Oral and Intravenous Glutathione, Microdermabrasion. Results are expected to be seen within 2-4 weeks depending on your response.
IV Glutathione
Glutathione is considered as the Master antioxidant, Its is used to treat patients with liver problems. Thru its use, it has shown to have skin whitening effects. The presence of glutathione shifts the melanin production to form more of pheo-melanin which is the light-colored pigment than eumelanin which is the dark colored pigment known as brown melanin. Giving one a lighter and fairer complexion. This can be given as an oral supplement or as an Intravenous treatment. Several sessions are required to produce effects.
Electrocautery
Electrocautery is a common treatment for benign skin outgrowths. Carried out with the use of topical anesthesia, Electrocautery carefully zaps each wart or benign skin outgrowth with a small burst of electrical energy. Technically, skin lesions are burned and the charred tissue are left to fall off naturally within a week or so. It doesn’t leave marks because most of the skin lesions are only superficial.

Chemical Peels serves many purposes. It can be used for Skin Rejuvenation; Acne scars, Melasma, Wrinkles, Active acne, Skin whitening. Different chemicals can be used such as Glycolic acid, Trichloroacetic acid, Salicylic acid, Lactic acid to slough off the surface of the skin.
Dead skin is peeled off to leave younger looking skin underneath. Skin is then protected from the harmful rays of the sun and treated with the appropriate home care regimen.
Varicose Veins

What are Varicose Veins?
Veins and arteries, while both part of the circulatory system, function quite differently from each other. "Poor circulation" is a nonspecific term which often refers to arterial blockages. Arteries bring oxygen-rich blood from the heart to the extremities and can be thought of like a tube or hose. Veins, unlike arteries, have one-way valves and channel oxygen-depleted blood back toward the heart. If the valves of the veins don't function well, blood doesn't flow efficiently. The veins become enlarged because they are congested with blood. These enlarged veins are commonly called spider veins or varicose veins. Spider veins are small red, blue or purple veins on the surface of the skin. Varicose veins are larger distended veins that are located somewhat deeper than spider veins.
Pain in the legs is frequently related to abnormal leg veins. Symptoms, often made worse by prolonged standing, include feelings of fatigue, heaviness, aching, burning, throbbing, itching, cramping, and restlessness of the legs. Leg swelling can occur. Severe varicose veins can compromise the nutrition of the skin and lead to eczema, inflammation or even ulceration of the lower leg.
Vein disorders are not always visible; diagnostic techniques are important tools in determining the cause and severity of the problem. In addition to a physical examination, non-invasive ultrasound is often used.
What Causes Varicose Veins?
Heredity is the number one contributing factor causing varicose and spider veins. Women are more likely to suffer from abnormal leg veins. Up to 50% of American women may be affected. Hormonal factors including puberty, pregnancy, menopause, the use of birth control pills, estrogen, and progesterone affect the disease. It is very common for pregnant women to develop varicose veins during the first trimester. Pregnancy causes increases in hormone levels and blood volume which in turn cause veins to enlarge. In addition, the enlarged uterus causes increased pressure on the veins. Varicose veins due to pregnancy often improve within 3 months after delivery. However, with successive pregnancies, abnormal veins are more likely to remain. Other predisposing factors include aging, standing occupations, obesity and leg injury.
How Phlebology Help?
Phlebology is the field of medicine that deals with vein diseases. It has been an established medical specialty in Europe for more than 50 years; serious interest in phlebology has developed over the past two decades in the United States. In 2005, phlebology was recognized by the American Medical Association as a medical practice specialty.
The American College of Phlebology was founded in 1985 and is the largest phlebology society in the United States. It was established to improve the standard of care related to disorders of the veins. Its members are physicians and other health care professionals with backgrounds in a variety of medical specialties who share a common interest and expertise in vein diseases and disorders.
When and How are Veins Treated?
The most commonly asked questions are: "Do veins require treatment?" and "What treatment is best?" Veins that are cosmetically unappealing or cause pain or other symptoms are prime candidates for treatment. There are two general treatment options: conservative measures, such as compression stockings, and corrective measures such as sclerotherapy, surgery and light source/laser treatment. In some cases, a combination of treatment methods works best.
Sclerotherapy
Sclerotherapy can be used to treat both varicose and spider veins. A tiny needle is used to inject the veins with a medication that irritates the lining of the vein. In response, the veins collapse and are reabsorbed. The surface veins are no longer visible. Sclerotherapy relieves symptoms due to varicose and spider veins in most patients. With this procedure, veins can be dealt with at an early stage, helping to prevent further complications.
You may need anywhere from one to several sclerotherapy sessions for any vein region. Depending on the type and number of veins being treated you may have one to many injections per session. Generally, normal activities can be resumed after sclerotherapy. Medically prescribed support hose and/or bandages may need to be worn for several days to several weeks to assist in resolution of the veins. The procedure, performed in the doctor's office, usually causes only minimal discomfort. Bruising and pigmentation may occur after sclerotherapy. Bruising typically disappears within 1-2 weeks. Although pigmentation almost always fades, it can last for several months. Scarring and other complications are rare.
Duplex-Guided Sclerotherapy
Ultrasound-Guided Sclerotherapy
This is an in-office treatment alternative to surgical stripping. With this technique and based on the clinical judgment of the doctor, sclerotherapy is done with either a liquid or "foamed" sclerosant, while the doctor visually monitors the vein on an ultrasound screen. This enables treatment of veins that can't be seen because they are below the surface of the skin and would otherwise require surgical removal.
What Result Can You Expect?
With the evaluation and treatment methods available today, spider and varicose veins can be treated at a level of effectiveness and safety previously unattainable. Regardless which treatment method is used, its success depends in part on careful assessment of the problem by a knowledgeable phlebologist. There are many underlying causes of varicose veins, so successful treatment of a particular vein problem does not necessarily prevent vein problems from recurring in the future.

How is Vaser Hi Def performed?
The procedure involves precisely performed, deep and superficial liposuction around muscle groups in the abdomen: the chest in men and the lower back and gluteus in women using ultrasound assisted lipoplasty (VASER).
The Vaser instrument selectively liquifies the fat and allows the surgeon to target specific fat layers to create the desired sculpted appearance of the muscles without damaging connective tissue.
What goes on during a VHD procedure?
The procedure is performed with tumescent anesthetic infiltration. The first step is to mark the muscle bellies that will be enhanced. In women, high definition liposculpture targets the serratus muscles, the obliques and the rectus muscle in the abdominal area as well as the gluteus and perispinal muscles in the lumbosacral curve. In males, the same abdominal muscles are targeted with attention also directed to the transverse tendinous intersections within the rectus abdominous muscle in order to create a washboard appearance. In addition, the pectoralis muscles are enhanced because of their contribution to the anterior topographical view.
The sculpting procedure begins with fat removal that creates a “picture frame" around the muscles to be enhanced. Next, the surgeon performs more precise sculpting that leaves behind small amounts of fat to amplify the underlying muscle tone.
What happens after a VHD procedure?
After the procedure, patients must wait about a month before returning to their exercise routine. The first evidence of the improved muscle definition can be seen after about two weeks, when the post-procedural edema has appreciably resolved.
What are the possible complications?
The risk of seroma is slightly increased with this procedure probably because it involves removal of the superficial fat.
Seroma is less likely with a traditional liposuction that primarily involves removal of the deeper layers of fat. However, if it occurs it is easily treatable in the office and does not adversely affect the final result of the high definition procedure.

Dr. Claudine S. Roura is a board certified dermatologist and is Fellow of the Philippine Dermatological Society and the Philippine Academy of Cutaneous Surgery. A graduate of the University of Sto. Tomas Faculty of Medicine and Surgery, she completed her residency training in Dermatology at the Jose R. Reyes Memorial Medical Center.
